Program Description:
At the USC UAP Children's Hospital Los Angeles, we have
a transition team composed of pediatric and adult
service providers (such as DR, WIA, ILC) who provide
consultation on service recommendations to ASHCN. The
purpose of the team is to enhance not only the quality
of transition services provided to ASHCN but to enhance
the capacity of service professionals providing services
in health care settings as this team works with other
teams from other facilities as well.
